Postprandial blood glucose, checked two hours after the start of a meal, should remain below 180 mg/dL. However, the most comprehensive measure of glycaemic control over time is the HbA1c test, which reflects average blood sugar levels over the preceding two to three months. Preventing Long-Term Complications
Sustained hyperglycaemia damages small and large blood vessels throughout the body, leading to microvascular complications such as retinopathy, nephropathy, and neuropathy, as well as macrovascular complications including coronary artery disease, stroke, and peripheral arterial disease. Regular screening is essential: annual dilated eye examinations, urine albumin-to-creatinine ratio testing, foot examinations for sensation and pulses, and comprehensive lipid profiles should be part of every diabetic patient's care plan. Strict blood pressure control (target below 130/80 mmHg) and statin therapy for lipid management are equally critical, as cardiovascular disease remains the leading cause of morbidity and mortality in the diabetic population.
The Importance of Self-Monitoring
Home blood glucose monitoring empowers patients to understand how food, activity, stress, and medications affect their sugar levels. I recommend that patients on insulin or sulfonylureas check their blood glucose at least two to four times daily, while those on stable oral regimens may check less frequently. Continuous glucose monitoring systems, now increasingly accessible, provide real-time glucose readings and trend data that can significantly improve glycaemic control when used appropriately. Every patient should maintain a log of their readings and bring it to each consultation so that treatment adjustments can be data-driven and precise.
