Hypertension, or high blood pressure, is often called the silent killer because it typically produces no symptoms until significant damage has occurred. Affecting nearly one in three adults in India, it is the leading modifiable risk factor for cardiovascular disease, stroke, and kidney failure. The good news is that hypertension is highly treatable, and with the right approach, most patients can achieve excellent blood pressure control. As a physician managing hypertensive patients daily, I believe that education and partnership are the keys to successful treatment.
Understanding Your Blood Pressure Readings
Blood pressure is expressed as two numbers: systolic (the pressure when the heart contracts) over diastolic (the pressure when the heart rests between beats). A normal reading is below 120/80 mmHg. Elevated blood pressure ranges from 120–129 systolic with diastolic below 80. Stage 1 hypertension is defined as 130–139 systolic or 80–89 diastolic, while Stage 2 hypertension is 140 or higher systolic or 90 or higher diastolic. A hypertensive crisis — any reading above 180/120 — requires immediate medical attention. I advise all my patients to know their numbers and to take multiple readings over time rather than relying on a single measurement, as blood pressure naturally fluctuates throughout the day.
The DASH Diet Approach
Dietary modification is one of the most powerful tools for lowering blood pressure. The DASH (Dietary Approaches to Stop Hypertension) diet has been extensively studied and proven to reduce systolic blood pressure by 8 to 14 mmHg. The diet emphasises fruits, vegetables, whole grains, lean protein, and low-fat dairy while limiting saturated fat, cholesterol, and refined sugars. A key component is sodium restriction: the standard DASH diet limits sodium to 2,300 mg per day, and a lower-sodium version restricts it to 1,500 mg per day. I recommend that patients read food labels carefully, avoid processed foods, use herbs and spices instead of salt for flavouring, and be mindful of hidden sodium in restaurant meals and packaged snacks.
Exercise and Physical Activity
Regular aerobic exercise can lower systolic blood pressure by an average of 5 to 8 mmHg. The American Heart Association recommends at least 150 minutes per week of moderate-intensity aerobic activity — brisk walking, cycling, swimming, or light jogging — or 75 minutes of vigorous activity. Consistency matters more than intensity; even 10-minute sessions spread throughout the day confer cardiovascular benefits. Resistance training twice weekly adds further benefit. For patients who have been sedentary, I always advise starting gradually and consulting a physician before beginning any new exercise programme, particularly if other cardiovascular risk factors are present.
Medication Classes and Treatment
When lifestyle modifications alone are insufficient, antihypertensive medications play a crucial role. First-line agents include thiazide diuretics, ACE inhibitors, angiotensin receptor blockers (ARBs), and calcium channel blockers (CCBs). ACE inhibitors such as ramipril and enalapril work by relaxing blood vessels, while ARBs like telmisartan and losartan block the effects of angiotensin II. CCBs such as amlodipine prevent calcium from entering heart and blood vessel muscle cells, leading to relaxation. Many patients require two or more medications to achieve target blood pressure, and combination therapy often allows for lower doses of each agent with fewer side effects. It is essential that patients take their medications exactly as prescribed and never discontinue them without consulting their doctor, even if they feel well.
Home Monitoring and When to Seek Help
Home blood pressure monitoring is invaluable for tracking progress and identifying white-coat hypertension (elevated readings in the clinical setting that normalise at home). I recommend an automatic, upper-arm cuff monitor validated for clinical accuracy. Patients should take readings at the same time each day — morning before medication and evening before dinner — after sitting quietly for five minutes with feet flat on the floor and the cuff at heart level. If home readings consistently exceed 135/85 mmHg, a treatment adjustment may be needed. Seek emergency care immediately if blood pressure exceeds 180/120 mmHg accompanied by chest pain, shortness of breath, severe headache, vision changes, or neurological symptoms, as this may indicate a hypertensive emergency requiring urgent intervention.
Controlling blood pressure is a lifelong commitment, but with the right strategies and support, it is entirely achievable. Regular follow-up with your physician, adherence to prescribed treatments, and a healthy lifestyle are the cornerstones of success.
